An unsafe environment can lead to a serious injury or death. Accidental falls in the home account for four out of five injury-related hospitalizations in older adults. 20% of all deaths among older adults are caused by falls. Each year 1.6 million older adults are treated in hospital emergency rooms for injuries caused by falls. Over 50% of falls occur at home as a result of slipping on a wet floor, losing balance on the stairs or tripping on a rug.

  Adequate hand washing facilities available  
  Medications are labeled and stored properly, safe from children, confused adult, or suicidal patients and are discarded when illness for which they were prescribed is over  
  Handrails for steps and stairs inside and outside  
  Loose rugs, runners, small mats, slippery waxed floors, and clutter removed to permit safe mobility  
  Adequate lighting is in each room with appropriate size and type of bulbs  
  Evacuation plan in case of fire/earthquake  
  Telephone accessible to client, emergency numbers posted nearby  
  Lifeline for individuals who live alone at risk of falling  
  Non-skid mats or emery strips in tub/shower; grab bars present
 
  Shower seat, transfer tub bench and raised toilet seat used  
  Smoke detector on each floor of home checked monthly for proper functioning; fire extinguisher present and charged  
  All flammables including heaters, oxygen and chemicals stored away from open flame  
  All cleaning fluids, polishes, bleaches, detergents, and other chemicals stored separately and clearly marked and away from range/oven  
  Electrical cords or telephone cords out of walkways  
  First aid kit available  
  Hot water temperature is 120 degrees or lower  
  Grease and liquids wiped up immediately when spilled  
  Food supply in the house is adequate and properly stored  
  Animals properly controlled and adequately kept
